Description

This grant-funded position will be responsible for providing education sessions to at least 300 patients per year through a structured, evidence-based curriculum delivered via technology in individual and/or group settings. The diabetes educator will also be responsible for entering appropriate patient data into a secure patient interaction tracking system, assisting with preparation of progress reports, and assisting with other grant-specific support duties. This will involve occasional nontraditional hours and weekend duties.

Minimum Experience Required: Bachelors degree; one or more years of related work experience (a higher level of education may substitute for some of the experience requirement); experience in designing, facilitating, and assessing appropriately challenging curriculum for diverse populations; experience with risk management, teaching and mentoring in an education environment; effective interpersonal skills to include flexibility and leadership; effective communication (verbal and written), organization, and human relations skills; ability to work well in a diverse environment; proficiency with computers and Microsoft Office applications software including word processing, spreadsheets, and databases; ability to work well in a diverse environment; successful completion of background investigation prior to employment.
